Transition Services
At Solor we recognise that people may need a variety of services during their life to reflect their changing needs. This is why we have a broad range of high-end residential services that are designed to be transitional to meet people’s shorter term needs until they are ready to move on to supported living or lower dependency residential settings.
There are many reasons that an individual may need to use one of our transition services, including:

As a step through from a residential college to a more independent supported living service

To support individuals to become more independent in a higher-intensity and lower-risk environment

Tailored support for adults with Autism or Aspergers requiring specialised services to assist with moving to a more independent environment

Change to mental or physical health, meaning more specialist input and support including nursing care is required for a period of time

Change in behaviour patterns that is not manageable in the current placement and support is required to stabilise these more challenging behaviours before a move on to supported living
